Northern Medical Group is proud to be a designated Patient Centered Medical Home (PCMH) receiving accreditation by the National Committee for Quality Assurance (NCQA). 

A Patient Centered Medical Home focuses on a comprehensive and coordinated team approach to provide quality care that meets our patient’s individual needs. Your medical home team includes your primary care provider, your support system and, most importantly, you!

At Northern Medical Group, patients and families are regarded as core members of the care team ensuring that patients are fully informed partners in their medical care. Your care team helps you to manage your health care needs by listening to your concerns, answering your questions, and working with you and your family to provide you with the best care possible. This includes:

  • Patient Education – the more you know, the healthier you can be 
  • Self-Management Support – helping patients with chronic conditions to take an active role in their health care
  • When necessary, we collaborate with your other chosen providers to ensure you are receiving the best quality care. 

Providing Continuity of Care

Patient Centered Medical Home is an innovative approach devoted to treating the whole person across many different areas of health care; to maintain continuity of care for every patient. 

You are an essential contributor to your health care. By providing us with an accurate medical history, as well as information about any care you have obtained outside of our practice, you ensure we have the complete picture. This includes sharing a list of current medications, recent test results, visits to specialty providers and hospital or emergency room visits.

Northern Medical Group provides extended access to medical care beyond normal business hours. When our offices are closed, a healthcare professional is always on-call and available for urgent matters 24/7. (Do they provide virtual assistance for the Deaf and hearing-impaired?)
